Who knows? Maybe sex is intrinsically a power exchange and bringing that to an extreme amplifies the experience. Maybe the act of sex is fundamentally paradoxical, for there is no benefit for the individual health, wealth or social standing in doing an act that will only benefit the society by producing new humans. If so the act of pain and pleasure are confused because the act of sexual pleasure itself should be none-pleasurably but is emulated by the brain to be so.
Maybe the act of being "forced"/"enslaved" into sex was so common during the evolution of mankind that it is no intrinsically linked to our social behavior. Therefore doing SM will activate ancient behavior patterns in the human brain that will release enough dopamine for the participants to bond as a family unit beside the social difference between slave and master.
Or maybe people just have a fetish. A very common fetish.
